How Should You Take Metaglip?

Special care may be needed.Patients over 65 years old may need a smaller dose than younger adults.Overdosage: If you think you have taken too much of this medicine contact a poison control center or emergency room at once.Note: This medicine is only for you. Extrapancreatic effects may play a part in the mechanism of action of oral sulfonylurea hypoglycemic drugs. Metformin hydrochloride is an antihyperglycemic agent that improves glucose tolerance in patients with type 2 diabetes, lowering both basal and postprandial plasma glucose. Patients previously treated with combination therapy of glipizide (or another sulfonylurea) plus metformin may be switched to Metaglip 2.5 mg/500 mg or 5 mg/500 mg; the starting dose should not exceed the daily dose of glipizide (or equivalent dose of another sulfonylurea) and metformin already being taken.

Low blood sugar is more likely to happen if you're older, weak, or undernourished, or if you have kidney, liver, adrenal, or pituitary gland problems. Your risk also increases if you miss meals or fail to eat after doing strenuous exercise. Combining Metaglip with other diabetes medications can also cause blood sugar to drop. Symptoms of a mild case include cold sweats, dizziness, shakiness, a light-headed feeling, and hunger. Check with your doctor immediately if you notice any of these warning signs, since severe low blood sugar can occasionally lead to seizures or coma.Before you start therapy with Metaglip, and at least once a year thereafter, your doctor will do a complete assessment of your kidney function.

Special Information If You Are Pregnant Or Breastfeeding

Metformin is known to be substantially excreted by the kidney, and the risk of metformin accumulation and lactic acidosis increases with the degree of impairment of renal function. An excessive dose of Metaglip can also trigger lactic acidosis. Follow your doctor's directions carefully. The usual maximum dose for adults is 2000 milligrams of metformin and 20 milligrams of glipizide each day. If you are already taking another anti-diabetic drug (e.g., chlorpropamide), follow your doctor's directions carefully for stopping the old drug and starting this combination medication.

Special Warning On Increased Risk Of Cardiovascular Mortality

Take this medication by mouth, usually once or twice a day with meals or as directed by your doctor. Drink plenty of fluids while taking this medication unless otherwise directed by your doctor. The dosage is based on your medical condition and response to therapy. Your doctor may direct you to take a low dose at first, gradually increasing the dose to lower the chance of side effects such as upset stomach and low blood sugar. Your doctor will adjust your dose based on your blood sugar levels to find the best dose for you.

Lactic acidosis is more likely to occur in patients who have certain medical conditions, including kidney or liver disease, conditions that may cause a low oxygen blood level or poor circulation (e.g., severe congestive heart failure, recent heart attack, recent stroke), heavy alcohol use, a severe loss of body fluids (dehydration), X-ray or scanning procedures that require an injectable iodinated contrast drug, recent surgery, or a serious infection. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.
